What Happened:
Recently, a third party sent in a vial of what was believed to be cagrilitinide to our lab for testing although the contents did not have any active ingredient in it. While most labs reported no detectable product, our initial test indicated a positive result. Upon reviewing our data, we discovered that, due to a human error, we had inadvertently copied the control sample used in the analysis instead of the actual sample submitted by our client.
Our Response:
As soon as we identified the mistake, we took immediate corrective actions:
- Client Notification: We reached out to the affected client, explained the error, and reassured them that accuracy is our top priority.
- Updated Certification: An updated Certificate of Analysis (COA) was issued, reflecting the corrected test results.
- Enhanced Procedures: We have revised our sample-handling and data entry protocols to ensure such an error cannot happen again.
- Free Retest Offer: As a gesture of goodwill and commitment to quality, we offered the client a complimentary retest.
